A severe food allergy is defined as a dangerous or life threatening reaction to a food-borne allergen introduced by inhalation, ingestion, or skin contact that requires immediate medical attention. If a licensed physician’s assessment indicates the student has food allergies which may result in severe, life-threatening(anaphylactic) reactions, the child’s condition would meet the definition of “disability,” and the substitutions prescribed by the licensed physician will be accommodated.

My child is on a special diet, what do I do if my student wants to eat at the school café?
Be sure you have a current allergy form signed by an approved medical professional and submitted to the Cafeteria Manager at your student’s school.
The Cafeteria Manager will then scan and send the form to the Child Nutrition Supervisor.
If the diagnosis is new, call the School Nutrition Supervisor at (435) 734-4800 to discuss any available accommodations.
If you are returning to the same school, touch base with your Cafeteria Manager at the beginning of the school year.
If you are NEW to that school or to Box Elder School District, please call the Cafeteria Manager to be sure all alerts are up to date, and then the School Nutrition Supervisor will direct you to the next step.
